If you just finished a year of Invisalign or had your braces removed and the first thing you noticed was chalky white spots after Invisalign exactly where your aligners or brackets sat — you are not alone, and the fix is far less dramatic than the panic suggests. These spots are demineralized enamel, they are mostly reversible, and there is a cheapest-to-most-effective treatment hierarchy that I run with patients every week at Soothing Dental.
I am Dr. Sona Saeidi, and what follows is the exact decision tree I use, written for a patient audience. We will cover why this happens, what to do at home before you spend any money in a dental chair, and the in-office options when home care is not enough.
Why white spots show up after orthodontic treatment
Two mechanisms. First, plaque retention — brackets, attachments, and aligner trays trap biofilm against the enamel surface. The bacteria in that biofilm produce acid every time you eat a carbohydrate, and the acid sits against the tooth instead of being washed off by saliva. Second, salivary access — Invisalign aligners and orthodontic appliances physically block saliva from reaching the enamel for most of the day. Saliva is the body's main remineralization vehicle, and when you remove it for 20 hours a day, the demineralization-remineralization balance tilts toward demineralization.
The result is a subsurface mineral loss that shows up as a chalky, opaque region with a glossy intact surface — a white spot lesion. Around the gumline and along the path of an aligner edge are the classic locations.
Are these cavities?
No, not yet. They are the precursor stage. Caught now, you can reverse early cavities and white spots without drilling with the protocol below. Left alone for years in a high-risk mouth, they can progress into frank cavities. The window for non-invasive treatment is the first 6–12 months after appliance removal, while the lesion is still active and responsive.
Treating white spots after Invisalign — cheapest to most effective
Step 1: Fix the home routine first (free, ~6 weeks)
Before you spend a dollar in the dental chair, run a clean home protocol for 6 weeks. Most mild post-orthodontic white spots after Invisalign respond to this alone.
- Switch to a 5,000 ppm prescription fluoride toothpaste, twice a day. Spit, do not rinse — let it sit on the enamel.
- Floss every night, especially around former bracket positions and proximal contacts.
- Stop sipping acidic drinks all day. Cluster coffee, tea, sparkling water at meals, then drink plain water between.
- Chew xylitol gum after meals you cannot brush after.
- If you still wear aligner-style retainers, take them out to drink anything but water.
Six weeks of this routine resolves a meaningful fraction of mild lesions on its own.
Step 2: Add MI Paste Plus (~$25, 8–12 weeks)
If the spots are still visible after 6 weeks of clean home care, add nightly MI Paste Plus. The CPP-ACP technology delivers calcium and phosphate to the enamel exactly when pH drops, and the 900 ppm fluoride drives mineral incorporation.
For post-ortho patients, the trick is to load a pea-sized amount of MI Paste into your existing aligner tray or retainer at night. The tray holds the paste against the teeth for the entire 7–8 hours, which is dramatically more effective than smearing it on with a finger. This is the single highest-value at-home intervention for white spots after Invisalign.
Step 3: In-office Curodont (~$200–400 per quadrant, single visit)
If the white spots are moderate or have not resolved after 12 weeks of MI Paste, Curodont peptide therapy is the next step. A single 5-minute in-office application places a peptide scaffold inside the lesion that recruits calcium and phosphate from saliva over the next 3–6 months. For most patients with multiple post-Invisalign spots, this is the inflection point — the procedure that produces visible regression their MI Paste alone could not achieve.
Step 4: Icon resin infiltration (~$300–500 per tooth)
If a spot remains visible at the smile line after Curodont and MI Paste, Icon resin infiltration is the cosmetic finisher. The resin penetrates the porous enamel and seals it; light then passes through normally and the spot blends into the rest of the tooth. Realistic timelines, in my experience:
- Mild spots, home routine only: faded but possibly still subtly visible at 6–8 weeks.
- Mild-to-moderate spots, MI Paste in tray + 5,000 ppm paste: meaningfully improved at 8–12 weeks.
- Moderate spots, Curodont + MI Paste: visibly regressed at the 3-month recheck, continued improvement out to 6 months.
- Aesthetic resolution at smile line: immediate after Icon, same day.
If you want the fastest aesthetic fix and you are post-Invisalign because a wedding, photoshoot, or job change is in three weeks — Icon today is the answer. If you have time and prefer the most conservative path, the home protocol plus MI Paste plus a recheck is the right sequence.

Preventing recurrence
The risk profile that produced the lesions does not disappear when the spots fade. If you continue to wear aligner-style retainers — and most Invisalign graduates do, indefinitely — you have to keep saliva access in mind. The retainer creates the same micro-environment that the original aligners did. The maintenance routine for retainer-wearing patients:
- Continue 5,000 ppm fluoride toothpaste long-term.
- MI Paste in the retainer tray 2–3 nights per week as ongoing maintenance.
- Take the retainer out to drink anything except water.
- Clean the retainer daily — biofilm on the inside of the retainer is biofilm against the enamel.
This routine is what separates patients who get a second round of white spots after Invisalign from patients who do not.

Will whitening get rid of white spots after Invisalign?
No. Whitening lightens the surrounding enamel, which usually increases the contrast and makes the white spots more visible. Treat the spots first, then whiten if desired.
How soon after Invisalign should I start treating white spots?
Immediately. The first 6–12 months after appliance removal is when the lesion is most active and most responsive to remineralization. Waiting years does not improve the outcome — it shrinks the window for non-invasive options.
Can I prevent white spots during Invisalign?
Yes. MI Paste in your aligner trays a few nights a week through active treatment, plus a prescription fluoride paste, plus disciplined removal of trays for any non-water drink, dramatically reduces incidence. Ask about preventive protocols at your next checkup.
Are post-orthodontic white spots permanent?
No. With the right protocol, the vast majority resolve or become visually indistinguishable from healthy enamel.
